What UHC covers
Addiction treatment is covered care, not an extra.
UnitedHealthcare commercial plans typically include coverage for medically necessary substance use and mental health treatment. That is the same category as any other medical care your plan covers. It is not a discretionary benefit you have to argue for.
Behavioral health under UHC is administered through Optum, which is why your card, your app or your benefits paperwork may say Optum rather than UnitedHealthcare. Both refer to the same coverage, and neither is a problem.
What differs between plans is the detail: deductible, coinsurance, how many days are authorized at each level of care, and whether prior authorization is needed. Those are the questions a verification answers, and they are worth answering before you decide anything.

UnitedHealthcare PPO plans are widely accepted across our facilities, and they are the most flexible kind of coverage to work with. The whole point of a PPO is that you are not confined to a narrow list of providers.
That includes out-of-network benefits. If your particular plan is not listed as in network at the facility you want, a policy with out-of-network coverage can typically still be used, and a good number of people who assume they are not covered turn out to be.
